Enrolment Procedure

For Incoming College Freshmen - Final Batch of Qualifiers
August 25 to September 4, 2020

  1. The security guard-on-duty forwards the submitted envelope to the Office of the Registrar.

  2. Please prepare your documents according to the following order, enclosed in a long brown envelope:
    • original or photocopy of Form 138;
    • original or photocopy Certificate of Good Moral Character;
    • photocopy of PSA Birth Certificate; and,
    • three pieces of 2x2 pictures.

  3. The enrolment for the final batch of qualifiers shall be from August 25 to September 4, 2020. For your specific schedule, please refer to the Enrolment Schedule in the next tab.

  4. Submit the same, enclosed in a long brown envelope, at the designated drop-point area (University Gate 2) for the Bacolor Campus  or the  main gate of the  extension campus with the following details:

                    Name of the Student: __________________________
                    Course and Year: _____________________________
                    Contact Number : _____________________________               

  5. The Registrar staff shall then enroll and inform the student regarding his/her section through text message.

Extension Campus - August 25 to August 28, 2020

Sto. Tomas Campus

 

Procedure in Requesting for Document

1.  The requesting student shall fill up completely and accurately the DOCUMENT REQUEST FORM and submits the same, enclosed in a long brown envelope, at the designated drop-point area (University Gate 2) with the following details:

            Name of the Student: __________________________

            Course and Year: _____________________________

            Contact Number :  ____________________________

            Date of Request: _____________________________

2.  The security guard-on-duty forwards the submitted envelope to the Office of the Registrar.

3.  The Registrar staff prepares the requested document and informs the requesting student of the date of release of said document and fees to pay.

4.  The student pays the necessary fees at the Cashier’s Office and claims the requested document from the Registrar’s Office.
