Admission Process for Senior High School

  • 1. Admission:
    1.1.1 The following requirements must be submitted prior to interview and before taking the exam as scheduled by the Guidance Office.
    • Duly accomplished application form;
    • Form 138 (report card) duly signed and certified by the Adviser and Principal;
    • PSA birth certificate (photocopy)
    • Two (2) identical copies of passport size recent photographs, (full-face latest ID) photo with background0 – affix name at the back;
    • Official receipt for testing and application fee amounting to Three Hundred Pesos (P300.00) to be paid at the Cashier’s Office – non- refundable;
    • Must present other evidence as the case maybe, graduation program or the certificate awarded to them.
    1.1.2. Present photocopy and original copy for authentication and reference purposes the following credentials for enrolment:
    The enrollee should be on the list of qualifiers posted by the SHS Principal:
    • Original copy of Form 138 (Grade 10 Report Card or its equivalent);
    • Philippine Statistics Authority (PSA) copy of Birth Certificate;
    • Original copy of Certificate of Good Moral Character from the Principal, Guidance Counselor, or Class Adviser;
    • Three (3) pieces of passport size identification pictures; • Enrollment Application Form;
    • Certificate of Education Subsidy Contract (ESC) indicating the Learning Reference Number (LRN) of Department of Education for those students who are recipient of SHS Voucher Program. (if necessary); and
    • QVR Certificate (if a recipient of the SHS voucher program) 2.1.2 Requirements for Admission for Transferees
    1.1.3 Foreign Students (In coordination with the office of International Affairs)
    The following are required from foreign students intending to study at the university:
    • Must have complete and valid credentials.
    • Must meet all the prescribed admission requirements of the University and the course applied for:
       b.1. University admission test
       b.2. Certification of completion of a secondary curriculum
       b. 3. Original transcript of records
       b. 4 Personal data
       b. 5. Affidavit of support
       b.6. Alien certificate of registration (ACR)
       b.7. Result of TOEFL
       b.8 Student visa
    1.1.4 Must be prescribed requirements by the Department of Foreign Affairs (DFA) and the Bureau of Immigration and Deportation (BID)

1.1.5 Must submit Certification of Proficiency in English based on TOEFL score (for non-native speakers of English)

1.1.6 A foreign student may be admitted based on availability of slot of the course applied for.

1.1.7 Admission of foreign students should be limited to specific courses.

      Step 1. Obtain Application Form through the authorized school representative or directly from the DHVSU Office of Admissions.
      Step 2. Carefully read the information and instructions then fill out the Application Form.
      Step 3. Register at the DHVSU Online Admission Portal , fill out the required information and submit all the requirements.
  1. 3. Qualification Process
    1.3.1. For the Academic Tracks such as:
    Science, Technology, Mathematics and Engineering (STEM) Strand
    Accountancy, Business Management (ABM) Strand
    Humanities and Social Sciences (HUMSS) Strand

    The following are the qualifications:
    • Must not have a grade lower than 80 in all subjects.
    • Must be of good moral character as certified by the Principal and Guidance Counselor.
    • Must pass the entrance examination and qualifying examination.

    1.3.2 For the Technical Vocational and Livelihood Track (TVL) such as:
              Electrical Installation and Maintenance
              Dressmaking
              Food and Beverage Services

    The following are the qualifications:
    • Must not have a failing grade in all subjects.
    • Must be of good moral character as certified by the Principal and Guidance Counselor.
    • Must pass the entrance examination.
    1. 3. 3 Qualified applicants will be chosen based on the following criteria:

College Admission

COLLEGE ADMISSION PROCESS

Step 1.
Obtain Application Form through the authorized school representative or directly from the DHVSU Office of the Admission and Testing.

Step 2.
Carefully read the information and instructions then fill out the Application Form.

Step 3.
Register at the DHVSU Online Admission Portal , fill out the required information and submit all the requirements.

Freshmen:
     Duly accomplished Application Form
     Photocopy of Form138
     2 pcs. passport size picture with name tag (white background)
     PSA Birth Certificate

Transferees:
     Duly accomplished Application Form
     Photocopy of Grades
     Copy of Honorable Dismissal
     Certificate of Good Moral
     2 pcs. passport size picture with name tag (white background) 

Graduate School Admission

Application and Enrollment Procedures for Graduate School Program

Step 1.
Register at the DHVSU Online Admission Portal , fill out the required information and submit all the requirements.

Requirements:
     Transcript of Records
     PSA Birth Certificate
     Marriage Contract (if applicable)
     2 pcs. passport size pictures with name tag(white background)
     Php 300.00 (Testing Fee)

Step 2.
Proceed to Window 2 of the Cashier's Office at the Admiistration Building for the payment of the Application Fee (P300.00) abd secure the corresponding Official Receipt.

Step 3.
Submit all the requirements to the DHVSU Office of Admission and Testing for the Graduate School Admission Test to be administered on the same day. Secure Test Result Slip.

Step 4.
Then proceed to the Office of the Dean of the Graduate School for securing and filling out of the Advising Slip.

Step 5.
Proceed to the Registrar's Office (University Library Building, 3rd Door) for the encoding of the subjects to be take and securing of an assessment form.

Step 6.
Psy tuition fee and other fees at the Cashier's Office

Step 7.
Secure Certification of Enrollement (COE) from the Accounting Office.

Step 8.
Present all pertinent documents to the Registrar's Office to secure class cards.

For OLD Students

        Step 1.
        Present ID and Clearance to the Office of the Dean of the Graduate School to secure Advising Slip.

Step 2.
Fill out the Advising Slip Form and proceed to the Registrar's Office for the encoding of the subjects to be taken and issuance of assessment form.

Step 3.
Pay tuition fee and other fees at the Cashier's Office.

Step 4.
Follow Steps 7 and 8.
