Enrollment Procedures 1st Semester AY 2021-2022

A. New Students (Incoming First Year)

  1. Visit THIS LINK for your specific enrolment schedule.
  2. Proceed to your respective campuses to submit your enrolment documents.
  3. Observe the DHVSU Task Force COVID-19 Guidelines once within the school premises.
  4. Proceed to the designated registration counters for the evaluation of the following documents:
    • Original copy of Report Card (Form 138);
    • Original copy of Certificate of Good Moral Character;
    • Photocopy of PSA Birth Certificate; and
    • Two (2) - 2x2 pictures.
      Note: Place the documents in a long brown envelope. Indicate the following information on the top left portion of the envelope:
             • Name of Student

             • Program
             • Contact Number
      The designated registration counters are in the following areas:
       • Bacolor Campus: Ground Floor, College of Education Building
       • Mexico Campus: Room 1 (Ground Floor), Academic Building
       • Lubao Campus: Room 105 (Ground Floor), Academic Building
       • Porac Campus: Room 3 (Ground Floor)
       • Sto. Tomas Campus: Room 1, Building 2
       • Candaba Campus: Registrar’s Office, Administrative Building
       • Apalit Campus: DHVSU Apalit Campus Office, Joggers Building Municipal Grounds, Apalit

       5. Proceed to the designated exit points.

B. Regular Students (2nd - 5th year Levels)

  1. The adviser shall require the students to submit a Letter of Intent (LOI) through their Gmail if they intend to enroll for the 1st semester of A.Y. 2021-2022.
  2. If in case the Gmail could not be accessed, the adviser shall require the students to accomplish the Letter of Intent Form. The final list of students who intend to enroll shall be submitted by the adviser to the chairperson. Then, the chairperson shall forward the final list of students to the Registrar’s Office.
  3. The record-in-charge of the program shall process the enrollment of the students.

C. Irregular Students

1. The record-in-charge shall provide a list of irregular students to the respective units.
2. The adviser/chairperson shall:
   2.1. check the Academic Program Evaluation at the student portal;
   2.2. contact the student for advising which will be carried out online; and
   2.3.  submit the peer-assessment forms to the dean for approval.
3. The chairperson shall forward the approved forms to the Registrar’s Office.
4. After which, the record-in-charge shall process the enrolment of irregular students.

D. Transferees/Shiftees/Returnees

  1. The student shall accomplish the online admission application provided by the Office of Admissions and shall submit the "Permit to Shift" Form duly signed by the parent/guardian together with the copy of grades from the last semester attended.
  2. The Office of Admissions shall endorse the student for evaluation to the respective unit where he/she intends to enroll.
  3. The Chairperson shall then evaluate the academic record of the student found in the Student Portal (for shiftees) and return the shifting/application form with remarks approved/disapproved for admission to the Office of the Admissions
  4. A duly accomplished advising slip shall be attached to the approved shifting/application form.
  5. The personnel-in-charge in the Office of Admissions shall endorse the application through the Online Admission Portal and forward the properly accomplished advising form to the Office of the University Registrar for appropriate action.
  6. The record-in-charge shall process the enrolment of the approved student-transferees and shiftees.

 The start of the 1st Semester for the College and the Senior High School and the Academic Year for Laboratory High School will be on September 14, 2021.

Download the DHVSU Health Declaration Form HERE. This form must be accomplished and dropped off at the University Entrance. If unable to print beforehand, said form is also available at the gate.
