B. Letter of Request for Review
C. Endorsement letter signed by the Adviser (for undergraduate and graduate research)
D. Proposal Study/Study Protocol
E. Technical Review Approval or Compliance Report
F. Curriculum Vitae of Researcher/s
G. Informed Consent Form (if applicable)
          a. English Version
          b. Filipino
          c. Local language
          d. Please specify: ______________
H. Assent Form (if applicable)

Step 2: Submit the Documents

A. Send PDF copy of document via PSUREC email: This email address is being protected from spambots. You need JavaScript enabled to view it. for the checking of completeness on the submission. Use the following subject line in your email: Application for Ethics Review_[Surname of the main proponent]
B. Provide a hard copy of all the documents submitted in a long brown expandable envelope to the Pampanga State University-Research Ethics Committee (REC) office.

Step 3: Check Email

Wait for a confirmation email regarding the status of your application. Notifications and further instructions will be sent to the email address you provided.
